Medical Forms

All Medicines must be handed in to the Front Office as they are responsible for storing them. 

We require the following form to be completed, which you can download below or copies are available at the Front Office.  

* Parental Agreement form - record of medication

Without the completion of this form we are unable to administer medicines. 

Please note that medicine will be returned for safe disposal when it has expired or at the end of each academic year.

Students with inhalers or autoinjector pens should keep their medication with them at all times. We can also store spares in the front office.

Parents should be aware that the School is not responsible for any incidents involving medicine on the way to/from School.
